Back in July, I completed a series of four Factor 4 treatments at the highly regarded Skin & Injecting Clinic on behalf of Dr Bryan C Mendelson. It was an incredible experience, but before I divulge just how effective this treatment was for me, let’s remind ourselves exactly what Factor4 is.

Factor4, an autologous conditioning serum (ACS), is a highly concentrated and super-rich blood serum, extracted from the patient’s blood. It’s like liquid gold to look at and it contains four times more growth factors than any brand blood-rich injection product or treatment available on the market.

When used in conjunction with Medical Skin Needling (also known as Collagen Induction Therapy), the area is treated with the body’s own potent growth factors. The result is faster healing and less down time.

Sometimes referred to as the Vampire Facial, the combination of Factor4 and Medical Skin Needling is powerful to say the least.

At the end of my last post I was a few weeks past my very first session with Dermal Clinician, Natasha Laouta. The reaction to my skin was surprising this early on. After a few intense days of redness and a little flaking, thanks to the Environ products provided as part of the treatment, my skin started to change dramatically and I still had three treatments to get through.

Heading into my second treatment, my nerves were steadier as I knew what to expect. The numbing cream was applied and I was in the chair ‘relaxing’ as Natasha took the Medical Skin Needling to the next level. Areas of concern like the open pores between my eyebrows and my concentration lines were focused on. I could feel the pressure, and it did result in a little bleeding, but it didn’t hurt.

Undergoing Factor4 in the cooler months is a blessing, if not an actual necessity. The fresh Melbourne air felt great on my warm post treatment skin as I left the clinic however by the morning, following use of the Environ products, the redness had significantly reduced.

Over the next four weeks I undertook two more Factor4 sessions (they can be done as closely as 5-7 days apart) with Natasha and each time she knew exactly the intensity required to achieve a good result. I always felt like I was in great hands. This combined with professional skin care products and the right medical make-up got me through the sessions with ease.

According to Natasha, results are at their best at five to six months post treatment, which is where I am right now, and I have to say I agree. My skin feels soft and smooth and the tone is still improving; glowing skin is not as hard to achieve as it was 12 months ago. In fact, I am no longer afraid to leave the house without makeup and I probably do it more now than I have in the past decade.

So, has turning back the clock made me want to throw in the routine and go to bed with my make up on like years gone by? Absolutely not! At home I now use a dermal roller and have stuck religiously to the Environ skincare routine Natasha started me on. In addition, there are maintenance programs of top up treatments at six months and annual boosters that I can take advantage of to continually improve my skin.

The Factor4 verdict

Overall, I would highly recommend Factor4 combined with Medical Skin Needling to anyone wanting to bring their skin back to peak condition. In fact, I would say don’t hesitate. So successful has been my treatment that a few friends and family are now planning their own visit to the Skin & Injecting Clinic for Factor4 treatments.

Hmmm, I usually love an infusion of sorts… tea infusions, alcoholic beverage infusions, (not necessarily blood infusions lol) but I have to say I’ve never heard of a dermal infusion. However, I’m always interested in anything Dr Bryan Mendelson’s Skin & Injecting Clinic is doing, and dermal infusions is where it’s at. Let’s check it out.

Plastic surgeons report growing number of people over 65 going under the knife

While the focus lately has been the number of young people opting for cosmetic surgery, statistics show there’s been a significant increase at the other end of the scale too.

According to the American Society for Aesthetic Plastic Surgery, the number of people aged 65 and older choosing to have facelifts and cosmetic eyelid surgeries has doubled over the last 20 years.

A closer look reveals a significant percentage of this increase has occurred in the last five years.

Last year, 37,632 facelifts and 39,772 eyelid surgeries were performed on people 65 and older, with three quarters of those patients totally new to surgery.

While there aren’t the same statistics available here in Australia, Dr Jack Zoumaras of Sydney’s Artiste Plastic Surgery says he has seen an increase in older patients.

“The average age of a patient requesting a facelift is in their 60s. As people live longer, older patients are having more plastic surgery,” he says.

“The most popular requests are a facelift and neck lift surgery.”

Other popular choices in the age bracket include breast implant changes in those who had them inserted when younger, eyelid surgery and non-surgical Botox and fillers.

He believes there are a number of explanations for this increase.

“Traditionally after turning 60 years old patients have wanted a face and neck lift but with the advent of non-surgical facial techniques this has delayed some patients getting surgery so they are presenting for surgery at an older age,” he says.

“Also older patients – and in particular the baby boomer generation in Australia – have disposable income after downsizing and selling assets. They feel it is time to do something personally for themselves and seek the expertise of a plastic surgeon.”

The Washington Post argues that a growing acceptance of cosmetic surgery as well as a reduction in surgery's costs and invasiveness has contributed to the shift.

Of course, there's the growing problem of age discrimination as more and more people remain in the workforce (and dating game) longer.

However undergoing such procedures at an older age does have some risks.

"Things like obesity, diabetes, high blood pressure and other medical conditions impact your recovery from surgery," says  Dr Zoumaras.

"This highlights the importance of seeking a qualified specialist Plastic Surgeon who is governed and accredited by the medical board to assess the patient holistically."

The movement has been dubbed by some as 'Grandma Plastic Surgery', a term Dr Bryan Mendelson of The Centre for Facial Plastic Surgery, Melbourne rejects.


"I don't like this term as it is demeaning and comes from younger people who do not have any perspective what it is like for this group of women," he says.

Dr Zoumras predicts the number of over 65s opting to go under the knife will continue to rise.

"As preventative medicine and lifestyle factors are impacting on life expectancy more and more people will present for plastic surgery at an older age," he says.

Should men ever get cosmetic surgery?

Growing older is inevitable. But actually looking your age has become more of a choice than an unalterable reality with increasing numbers of men seeking out professional help to keep themselves looking the right side of 50.

But is anyone buying what these blokes are putting down?


Earlier this year, Tom Cruise set tongues and Twitter wagging when he appeared on the red carpet at the BAFTAs. The Mission: Impossible star was looking decidedly puffy in the face.

While Cruise has not confirmed any nips or tucks, rumours abound that the 54-year-old-actor (who is arguably in better shape than myself and many of my peers) had undergone a round of 'fillers' – injectables that help reduce the sign of wrinkles by literally filling them in from underneath.

Quick fixes

The fact that Cruise may have succumbed to cultural pressures to look younger than he is wasn't the issue. What was so shocking was the drastic difference in his appearance, with one commentator describing him as "a hamster in a tuxedo".

According to Dr Bryan Mendelson from the Centre for Facial Plastic Surgery in Melbourne, part of this is because men tend to wait until the damage is already done before they try to address it.

"It is generally said ... that male attractiveness peaks at a much later age than it does for women," Mendelson says.

"This may explain why men seem to worry less about the aging process until it is well established, which presumably explains why in general they have surgery performed at a later age."

Cathriona Flanagan, head dermal therapist at Sydney's Face of Man, says that Cruise's mistake is a common one made by many men opting for a tune up.

"Older men are looking for a fast fix rather than a slower, more subtle approach to getting that fresh faced look," she says.

The uncanny valley

Another reason that Cruise, and many other men trying to turn back the clock, fail to be praised for looking so surprisingly fresh and young is just that – it's a little too fresh, a little too young and a little too sudden.

There's a term in the world of technology and robotics called the uncanny valley. It's a term used to describe the point where a robot becomes so realistic it causes a sense of revulsion in humans.

It's a similar experience when we encounter someone who's gone from looking like a healthy bloke in his mid-50s to, well, a hamster in a tuxedo overnight.

Aged, like whiskey

Another reason is that we've been culturally hard-wired to prefer men who a look a little more weathered and tested by time. In comparison to women, who unfairly suffer from a ridiculous pressure to remain ever youthful, men are actually encouraged and celebrated as they look older.

A quick look at those men who consistently appear on "world's sexiest" lists would prove this theory, with most of the gents taking out top spot sporting more than just a smattering of silver in the hair.

Men like George Clooney, Matt Damon, Idris Elba and even Russell Crowe regularly position quite high in the hierarchy.

"We prefer our men to keep their masculine features, which is what some of these cosmetic procedures tend to alter," explains Flanagan.

"For example, an over use of filler in the cheekbone area. This is considered a beauty trait in females but something we are seeing more of in men."

So is there a happy medium?

Choosing to take the edge of the aging process isn't a bad thing - who doesn't want to look their best until the very end? But there are ways to do this that won't leave you looking like you're cosplaying Benjamin Button.

"The best advice I can offer someone would be to start doing skin rejuvenation treatments such as fractional laser or skin needling and good quality homecare (including sunscreen) as a means for preventive," says Flanagan.

"You don't develop permanent wrinkles until your mid to late 30s or even early 40s, so I wouldn't recommend Botox earlier than this."

"Most patients in Australia are just wanting to 'look good for their age'," explains Dr Mendelson. "This means looking healthy and vital. There is a phenomena we call the 'Melbourne look'. That is, natural and subtle, not obvious.

"Get good advice, which may require visiting a couple of recommended prospective surgeons to find the surgeon who specialises in surgery of the face and who you feel comfortable with; and prepare yourself for the surgery with good health, fitness, and especially weight control."

Bright Eyes – Advanced Blepharoplasty with Dr Bryan Mendelson

Up until very recently it was thought that tightening the skin was the best method for smoothing out those wrinkles and lines that come with ageing. However this is not necessarily the case. Dr Bryan Mendelson, Plastic Surgeon and internationally recognised authority on facial anatomy and ageing, uses a procedure he calls “Bright Eyes”, or Internal Fixation, being lauded as the most natural and lasting technique around today.

Internal Fixation

Blepharoplasty has helped thousands of patients restore and refresh the eye area to turn back the clock. The Internal Fixation technique is where the skin is brought back to where it was before it started to sag, creating a more distinct fold above the eye. The Internal Fixation technique is said to give completely natural looking results as the skin does not appear pulled or overstretched. Dr Mendelson clarifies why, “The natural shape of the eye forms the basis for the internal fixation with the result being that the eyes look bright and full of life. There is only one small incision, carefully concealed in the eyelid crease, which is undetectable within four months, even when the eyes are closed.”

Blepharoplasty, or eye surgery, is one of the best procedures for turning back the clock a few years. Eyes are one of the first areas of the face to show ageing. The procedure can offer other benefits such as:


  • Restoring vision by lifting any skin that drapes over the eyes;
  • Reduce any fat deposits that give the eye a puffy appearance beneath the eyelid;
  • Help reduce bags under the eyes;

For those of you who haven’t been paying attention, Factor4 is fast becoming one of the most talked about treatments of 2016. PSH: Does the process hurt, and how long does it take?

Each treatment takes approximately an hour and a half. This includes time for the numbing cream to take effect, followed by the skin needling procedure with the Factor4 serum and 13 minute LED therapy which assists with healing.

Due to the numbing cream, patients feel little more than light scratching on their skin. We also gauge the intensity on the client’s comfort levels as some areas of the face are more sensitive than others.

PSH: The results are said to improve over time. What is the recommended number of treatments (we hear 4 – 6 treatments will be needed with 2 or so weeks in between each treatment)? How long do the results last and how often do you need “top ups”?

A minimum of 4 treatments is generally required to achieve the optimum rejuvenation of the area. Recent research has resulted in a recommendation that skin needling treatments should be performed at weekly to fortnightly intervals to achieve the best possible results.

The true result of the treatment will be apparent approximately 3-6 months post treatment as your skin regenerates and continues to produce more collagen and elastin. Top ups are recommended every 6 months or annually depending on the condition and quality of the skin. A maintenance program can also be considered ie. an additional 4 treatments 6 weeks apart.

PSH: Are there any risks associated with the procedure and is it suitable for everyone?

Being an autologous serum it contains no cellular matter, limiting the risk of infection as well as no risk of bacteria or cancerous cell transfer.

Following the treatment a mild inflammatory reaction occurs, resulting in a warm, flushed feeling similar to a moderate sunburn. Skin redness can persist for up to 24 – 48 hours. It is normal for the skin to feel tender and in some instances mild bruising and swelling around the eyes and lips can be noticed.

This treatment is suitable for everyone particularly those with fine lines, dull skin, enlarged pores, acne scars, chicken pox scars and surgical scars, nasolabial fold lines, smoker’s lines and those seeking rejuvenation of hands, neck and decolletage, lax skin on the body – arms, legs and stomach. It is NOT suitable over skin cancer and active acne or during pregnancy.

PSH: It is said the most popular place to have the treatment is on the face, but it is possible to have the treatment almost anywhere on the body. Where do you find you are performing the treatment most and where are you getting the best results for the treatment?

We perform most of our treatment on the face, neck and decolletage and back of hands. The best results we have seen have been the neck, under the eyes, and nasolabial fold. In addition to visible appearance there is a softness of fine lines and pores.

PSH: Talk us through how the actual procedure works and what is happening to our skin in order for the treatment to work as well as it does.

The treatment involves the use of an electrical device called MDerma which uses tiny needles to create micro-injuries to the skin. These micro injuries induce a natural wound healing response which includes the release of growth factors and the deposition of new collagen and elastin to heal the skin. What Have You Done To Your Skin? Diary of A Factor 4 Treatment


Diary of a Factor4 treatment – Part 1 of 2.
Have you ever looked at your skin and wished you could turn back the clock, even just a few years? Have you wished that you could bring back the soft, healthy skin you thought you’d have forever? Well, me too, so I decided to see if it was possible.
While I have worn SPF 30+ religiously, years of sporadic skin care routines and lack of regular maintenance seemed to have finally caught up with me. My skin was looking dull, pores and lines were becoming more visible and, no matter how regularly I exfoliated, my skin tone looked blotchy.
Determined to find a solution, I spent months talking to friends and researching treatments around the world, just to see what was on offer.
My research took me on a journey from home skin needling and smoothies to Fraxel laser and chemical peels. While I flirted with the idea of Fraxel, I have to admit it was the downtime, more so than the cost, which made me put it on the back burner (so to speak).
However, it was a conversation with a friend (with beautiful skin) that lead me to reconsider a treatment called Factor4. “Why would you go through Fraxel when there is Factor4?” she asked. Like many others it seems, I thought Factor4 was the domain of high profile celebrities only (think Kim Kardashian and Bar Rafaeli); inaccessible to us mere mortals. I had no idea it was available and being practised successfully here in Melbourne.
To many, Factor4 is another name for the ‘Vampire Facial’ but according to my research through the Skin and Injecting Clinic in Melbourne, under the Direction of Dr Bryan Mendelson, it is incredibly effective at rejuvenating skin; delivering tone, texture, brightness and elasticity with minimal downtime. Here’s why…
Factor4, an autologous conditioning serum (ACS), is a highly concentrated and super-rich blood serum, extracted from the patient’s blood, which contains four times more growth factors. When used in conjunction with Medical Skin Needling (also known as CIT or Collagen Induction Therapy), the area is treated with the body’s own potent growth factors. The result is faster healing and less down time.
In a nutshell, a combination of Factor4 and Medical Skin Needling, is powerful. The treatment can seem extreme as occasionally patients do bleed, (hence the Vampire facial moniker) but this is not felt and is more like a controlled exfoliation than anything violent.
So, aside from a little blood, what did I have to lose? I booked an appointment with Natasha Laouta, Dermal Clinician at the Skin & Injecting Clinic immediately.
PRE-TREATMENT
I’m told that I will need four treatments, no more than two weeks apart, and will also need to prepare my skin with a home dermal rolling along with Vitamin rich oil A & C for at least a month before we begin. The commitment doesn’t put me off, in fact, I am excited by the opportunity of seeing this unfold gradually. As part of the Factort4 package, I am handed a skin care kit (including dermal roller) and off I go to have my blood extracted.
Four weeks later, I arrive for my first Factor4 treatment. I’ll admit I am nervous and tell Natasha so as she applies numbing cream over my face.
Having a fair and sensitive skin, which is now bright red in response to the numbing cream, I seek reassurance that I will be able to carry on as normal the next day. “Absolutely,” she says as she hands me a pre-arranged mineral makeup bag. I’m told that most of the redness will go down after a few hours, and while I’ll still be a little swollen and flaky, the makeup will help me get through the day.
After an hour, the numbing cream is removed and Natasha shows me a vile full of yellow liquid and explains that this is the substance, extracted from my blood, that will provide the powerful healing. To be applied to my skin during the Medical Skin Needling, I have four of these (three now in freezer storage) to get me through the treatments.
With my skin cleaned meticulously, we begin. Due to the hour spent covered in numbing cream, I can feel little more than slight scratching on my skin. Natasha works thoroughly across my face, concentrating on problem areas (fine lines and stretched pores) and applying the serum as she goes. I don’t bleed at all and once complete, I spend twenty glorious minutes under the warmth of the Low-level light therapy (LLLT) Healite II unit which aims to assist recovery, and a rich application of Environ Vitamin Oil containing A, C & E completes the session.
Before I know it, I am done and as I prepare to leave I grab a quick look in the mirror. My face is bright red and hot, and I look like I have a terrible sunburn. Thank goodness for the mineral make-up (BB Cream, powder and blush) provided as part of the treatment as I know I’ll be able to cover up tomorrow if I need to.
Fortunately, it is winter, and I am leave in the dark, but I do have a family dinner to go to, and while I am comfortable admitting what I have done, their reactions to my face are going to be interesting.
As I walk through the door of my mother’s house, all eyes land on me, and there is a collective gasp from the family. “What have you done to your skin?” my Mother asks, very concerned. I explain the procedure, and aside from offers of a cold press and comparisons to a lasered Samantha from Sex in the City, there isn’t too much fuss and they lose interest.
At home, I follow the skincare instructions carefully before jumping into bed for some cuartive sleep (on my back of course). I am very excited to see the changes ahead.
POST FIRST TREATMENT
On waking, the first thing I do is touch my face. My skin feels tight and slightly rough to the touch which is incredible considering how much oil I had put on it the night before.
I can’t wait to see what I’ll be dealing with today, so I head to the bathroom and inspect every inch of my skin. I’m pleased, and incredibly surprised, to see the redness has calmed down a lot. It’s still very blotchy and dry but with help from the skincare and mineral makeup provided, I think I can get away with it.
Day two and day three I continue to follow the skincare routine religiously. I am still slightly red and a little swollen without makeup, but I know things are improving. The one improvement I do notice is that my skin is starting to feel incredibly soft.
By day four I’m in a routine of skincare and mineral make-up and to be honest, I’m not sure I can see much of an improvement. That was until I was talking to a colleague at work. Mid conversation he interrupts me and asks, ‘what have you done to your skin?’ I am slightly worried that I am flaking but when he adds, ‘your skin looks amazing! Have you had a facial or something?’ I get flushed with excitement.
If this is the result after just one session, I can’t wait to get through the next three!
